Community Based Residential Facilities can meet the support needs of individuals requiring 24 hour/7 day per week supervision and other support needs. Our Wisconsin-licensed Community-Based Residential Facilities (CBRFs) are able to support five or more non-related individuals who require such services.

Since 1972, the Lori Knapp Companies have been providing residential services to children, people with disabilities, and persons who are elderly across Wisconsin. We take pride in seeing these folks have the same opportunities that other people have. We like to think of our certified and licensed Community-Based Residential Facilities (CBRFs) as places to experience "living".

The Lori Knapp Companies were the first in the Midwest to allow people with developmental disabilities to leave the institution and to live in their home communities. Since our first eight-bed group home was established, Lori Knapp Companies have been leaders in the field of residential housing. We believe that all people have the right to productive lives, and the Lori Knapp Companies strives to help our customers maintain their dignity and community life, no matter what their disability or age. Each of the people that we serve is treated as an individual, and not just one of large number. Care is taken to assess the needs of each person, and to provide as much personal service as possible.

Overview

Devoted to quality, each of our Community-Based Residential Facilities (CBRFs) are fully staffed with trained and competent caregivers. We are a licensed Assisted Living program through the State of Wisconsin. Being licensed in this fashion allows for an extra set of eyes on us to help ensure that we are quality-based. Some of our CBRFs provide 24-hour awake staff. In any CBRF, our model of care is person-driven and we employ an Individualized Service Plan (ISP) to help keep quality of care consistent. We strive to keep a "family-style" structure to our CBRFs. We recognize and validate that any of our assisted living programs are, in reality, a place where each person served would consider it to be their "home." How will my home be staffed and will it be "home-like"?

Supervision: The services provided are tailored to meet the individual's supervision, care, & support needs. Lori Knapp Companies will be responsible for managing the staffing patterns at the home and ensuring that staff are trained, qualified, and have had criminal background checks completed prior to working in any Assisted Living home. Home-Like Environment: We take pride in the fact that residences within the Lori Knapp Companies system are "homes". Although we're a business, we work hard to keep a home-like environment for our Assisted Living homes. We strive to have as much of a "least institutional" feel and style as possible. Furnishings are provided, but personal items & belongings are encouraged, so personal living space is unique and special. Any Lori Knapp Companies Assisted Living home is more than just a "house"… it is a unique "home." Accommodation Options: Each of our Assisted Living homes have unique amenities and features that help us provide quality services in a comfortable homelike environment. Depending on availability we offer private rooms and semi-private rooms. The list of amenities that we can provide is best learned by contacting us today. How do I get more info or start the admission process?

Individuals receiving public funding will need to coordinate with their care management team to start a referral process. To best serve you, we wish to know you and your needs. Through simple discussion, we can begin the process of learning with a no-cost, no-obligation telephone call or face-to-face meeting. We will work with you and other applicable experts to meet your individual needs. All aspects of your care are meticulously assessed and a plan is developed specifically for you.

How much will it cost me to live here?

Whether you are privately paying or accessing services through public funding, insurance, or other funding, the Lori Knapp Companies will work with you to help you receive the best care you deserve at costs you can afford. Charges are structured on a per-day, per-care level formula. Our rates are comparable to other care providers. If you are eligible for government funding, we will refer you to the right agencies for help. These include ADRCs, County Human, and Social Service agencies.

Who will be my Caregivers?

Our Lori Knapp Companies caregiving teams will provide consistent, quality cares that encourage dignity and safety. They are trained to provide quality care for persons with unique and special needs. The core team includes a staff of trained direct support workers to provide for your support needs, as well as a Residential Coordinator to supervise all caregiving efforts and activity. To promote consistency and quality, mandatory trainings are provided to meet the changing needs of the people we serve. Because the Lori Knapp Companies serves many different types of people, our trainings cover many different areas, including: safety & well-being, privacy & confidentiality, client rights, prevention of abuse and neglect, medication oversight, diabetic care, specific dietary needs, non-violent crisis intervention for persons with challenging behaviors, seizures, promoting health and wellness, needs and assessment, supervised on the job training, and additional state-mandated trainings. If there are special care needs, your staff will be trained to meet those needs. Having adequate team members on-site to serve you is the key to success. Each Assisted Living home is staffed to meet the individuals supervision and care needs. In some instances and areas around the state of Wisconsin, nursing supervision is requested. As such, the caregiving team may include an RN supervisor.

What about Individualized Service Plans (ISPs)?

Each person who lives in a Lori Knapp Companies CBRF will have a person-centered Individualized Service Plan (ISP) developed, implemented, & reviewed periodically. Person Centered means that the individual and their support system will drive the goals and outcomes of the ISP.
